- Complete Direct Deposit Authorization form
- Return the authorization form to the person in your department who handles payroll processing, who will forward it to Payroll Services.
- Contact Prudential Long Term Care Customer Service Center at 1-800-732-0416 to request the Enrollment Kit
- Return completed enrollment form to: Prudential Financial, P.O. Box 8526, Philadelphia , PA 19101-8526

Public Employees’ Retirement System (PERS)
- Complete PERS Enrollment Application
- Return the application to the person in your department responsible for payroll processing, who will forward it to Payroll Services.
Police and Firemen’s Retirement System (PFRS)
- Complete Enrollment Application
- Return the application to the person in your department responsible for payroll processing, who will forward it to Payroll Services.
Alternate Benefit Program (ABP)
- Complete an ABP Enrollment Application
- Complete an ABP Carrier Election and Allocation form
- Obtain unique enrollment form from state-authorized investment carrier
- Return unique enrollment form directly to carrier
- Return a copy of the unique enrollment form, ABP Enrollment Application, and ABP Carrier Election and Allocation form to the person in your department responsible for payroll processing, who will forward them to Payroll Services.
Flexible Spending Accounts
- Complete an Election Form
- Mail or fax the completed election form to Fringe Benefits Management Company (address and fax number on Section 1 of election form)
Premium Option Plan (POP)
- To waive participation, complete POP Declination Form
- Return POP Declination Form to the person in your department responsible for payroll processing, who will forward the form to Payroll Services.
- Complete ABP Salary Reduction Agreement
- Complete ABP Carrier Election and Allocation form
- Obtain unique 403(b) Enrollment Form from state-authorized investment carrier (TIAA-CREF participants complete Group Supplemental Retirement Enrollment Form)
- Return unique 403(b) Enrollment Form directly to carrier
- Provide a copy of the unique 403(b) Enrollment Form, ABP Salary Reduction Agreement, and ABP Carrier Election and Allocation form to the person in your department responsible for payroll processing, who will forward them to Payroll Services.
Additional Contributions Tax Sheltered Program (ACTS)
- Complete an ACTS Salary Reduction Agreement
- Complete ACTS Carrier Election and Allocation form
- Obtain unique 403(b) Enrollment Form from state-authorized investment carrier. (TIAA-CREF participants should complete a Group Supplemental Retirement Enrollment Form)
- Return unique 403(b) Enrollment Form directly to carrier
- Return a copy of the unique 403(b) Enrollment Form, ACTS Salary Reduction Agreement, and ACTS Carrier Election and Allocation form to the person in your department who handles payroll processing, who will forward them to Payroll Services.
Supplemental Annuity Collective Trust (SACT)
- Complete a SACT Enrollment Application
- Return the application to the person in your department who handles payroll processing, who will forward it to Payroll Services
