Life Events - Divorce/Dissolution of Civil Union or Same-Sex Domestic Partnership
Changes to records and benefit coverage levels may be appropriate as a result of your divorce or dissolution of a civil union/same-sex domestic partnership. Note that some changes must be reported within 30 days of the divorce decree or dissolution.
- Update your address and emergency contact information
- Notify the university of a name change
- Change your federal withholding allowance
- Change health insurance coverage
- Update beneficiaries for life insurance and investment accounts
- Change flexible spending contribution
- Change Long Term Care coverage
- Notify the Credit Union of change in marital or same-sex domestic partner or civil union status
Update Home Address and Contact Information
- Change your home address, phone number, and emergency contact information online through the Address Directory Record (ADR). Make your ADR changes immediately as this information is used for W-2 mailing.
- To record your name change, a Personnel Data Record (PDR) form must be completed by the person in your office who completes payroll forms.
- A name change also requires that you complete a new W-4 form.
- You may also want to change your name through the Social Security Administration.
Instructions for changing federal withholding allowance are available from the Division of Payroll Services. W4 forms may be obtained through the individual in your office who processes payroll forms.
To cancel coverage on your former spouse:
- Complete the NJ State Health Benefits Program Medical Application.
- Complete the NJ State Health Benefits Program Dental Application.
- Return the form(s) to the person in your department in charge of payroll/forms processing within 30 days of the divorce or dissolution decree
- Notify University Human Resources of your divorce or dissolution. We will send a COBRA notification to you or to your former spouse or civil union partner/same-sex domestic partner
- Your former spouse or civil union partner/same-sex domestic partner may continue medical, dental, prescription drug and vision coverage for up to 36 months under COBRA

All Pension Plan Participants
- Update life insurance beneficiary information by completing a Designation of Beneficiary form and mailing it to the State Division of Pensions and Benefits.
ABP Retirement Accounts
- Update beneficiary information for investment accounts. Contact the appropriate investment carrier for a Designation of Beneficiary form:

Flexible Spending Accounts (Divorce only)
You may elect to:
- Enroll in the Unreimbursed Medical or Dependent Care Flexible Spending Account
- Increase or decrease your Annual Salary Deduction amount
- Cancel coverage
- Make no changes (no action necessary)
To enroll, or change your Annual Salary Deduction amount within 30 days of the divorce decree:
- Complete the Unreimbursed Medical/Dependent Care Spending Account Change in Family Status While Actively Employed form.
- Return the completed form to University Human Resources
To enroll or to change Long Term Care coverage:
- Complete the Prudential Long Term Care Insurance “Long Enrollment” application. You may enroll online, or call Prudential at 1-800-732-0416 to obtain an application.
- Return the completed form to:

- Notify the carrier, including your name, policy/certificate number, and the date you wish to terminate the policy.

- To remove your former spouse or civil union/same-sex domestic partner from your account, the account must be closed and a new account opened in your name. Contact the Rutgers Federal Credit Union for additional information.
