Long Term Care Insurance
State of New Jersey Long Term Care Insurance Plan
Long Term Care Insurance is a voluntary program administered by Prudential and covers a broad range of services for anyone who has lost ability to function independently.
The cost is determined by your age at the time of enrollment. Payroll deduction is available.
- Enrollment Eligibility
- Employees, retirees and the following eligible dependents: spouse; same-sex civil union or domestic partner; adult children and stepchildren; parents, step-parents and parents-in-law; grandparents, step-grandparents, and grandparents-in-law

- Enrollment
- New employees who enroll within 90 days of hire use short form with minimal medical questions. If enrolling after 90 days, the long form must be completed

- Coverage
- Daily benefit amounts toward care in the home and community, such as home health care, adult day care, informal care, and care that is provided in licensed facilities such as assisted living and nursing homes
