PERS and PFRS Long Term Disability Insurance

Who is Eligible:

Certain full-time Administrative and Managerial, Professional, Supervisory, and Confidential employees of the university enrolled in PERS & PFRS. The employee must:

• Not be covered by a collective bargaining agreement, except members of a Federal Retirement System
• Hold at least a 10-month appointment
• Receive an annual salary of at least $15,000

Enrollment:

You must enroll at the time of your initial eligibility. If you enroll after that period, proof of insurability must be submitted. You are initially eligible when:

• You are a new hire (coverage is effective the first day of the month following a full three-month waiting period)
• You have a job change from an ineligible position to an eligible position (coverage is generally effective the first of the month following the job change)

Benefit Amount:

66 2/3% of monthly earnings not to exceed $5,000 per month

How to Apply:

Complete a UNUM LTD Enrollment Form. If you are enrolling 31 days after your eligibility date, an UNUM Provident LTD Evidence of Insurance form must be completed. Submit form(s) to the person in your department who handles payroll processing.
