Program Overview
- Administered by Caremark
- Over 95 percent of pharmacies nationwide participate
- Most pharmacies in New Jersey participate
- $3 co-payment for up to a 30-day supply of a generic drug
- $10 co-payment for up to a 30-day supply of a brand name drug where no generic equivalent is available
- $25 co-payment for up to a 30-day supply for brand name drugs where a generic equivalent is available
- Mail order: $5 co-payment for up to a 90-day supply of generic drugs
- Mail order: $15 co-payment for up to a 90-day supply of brand name drugs where no generic equivalent is available
- Mail order: $40 co-payment for up to a 90-day supply for brand name drugs where a generic equivalent is available
- Specialty Pharmaceuticals administered by Caremark Specialty Pharmacy Services
Specialty Pharmaceuticals
Caremark Specialty Pharmacy Services is the exclusive provider for specialty pharmaceuticals. "Specialty pharmaceuticals" are medications that are typically produced through biotechnology and are usually administered by injection and/or require special patient handling and monitoring. Employees must contact Caremark Connect to enroll in the program to receive specialty pharmaceuticals.
