The New Jersey Division of Pensions and Benefits announced changes to the State Health Benefits Program (SHBP) effective July 1, 2007. These changes include a new health and/or prescription insurance contribution and copayment amounts.
Forms necessary to waive your SHBP insurance coverage:

- The standard employee contribution set forth in the law for an employee enrolled in the SHBP for medical (NJ Plus, Traditional Plan, any HMO) and/or prescription coverage is 1.5 percent of his or her annual salary, regardless of level of coverage (e.g., single, family) or type of coverage (e.g., medical coverage, prescription drug only coverage).
- The university began withholding this insurance contribution in September for some employees. See the FAQ page for additional information on the withholding dates for various employee groups. As required by the state, contributions are retroactive to July 1, 2007 for 12-month employees and retroactive to September 1, 2007 for 10-month employees.
- Employees have the option to waive their insurance coverage provided they can demonstrate other coverage. To waive your SHBP coverage, both the SHBP Waiver/Reinstatement form and a SHBP Benefit Application must be completed and sent to the Division of Payroll Services.

- For NJ Plus and all HMO subscribers, the office visit copayment for primary care physicians and specialists is now $15.
- The copayment for an emergency room visit is now $50, although the copayment is waived if the member is admitted to the hospital.
- Prescriptions purchased through a pharmacy (up to a 30-day supply):
- The cost of a brand name drug prescription where a generic alternative is available is now $25.
- The cost for generic drugs remains $3.
- The cost for brand name drugs where no generic alternative is available remains $10.
- Prescriptions purchased through the mail order system (up to a 90-day supply):
- The cost of a brand name drug prescription where a generic alternative is available is now $40.
- The cost for generic drugs remains $5.
- The cost for brand name drugs where no generic alternative is available remains $15.
