Vision Care Plan
Overview
You may receive $45 reimbursement for single vision corrective lenses and receive $50 reimbursement for bifocal or trifocal corrective lenses purchased on or after December 15, 2007.
Purchases made prior to December 15, 2007, will be reimbursed at the rate of $35 for single vision corrective lenses and $40 for bifocal or trifocal corrective lenses.
Employees and eligible dependents are entitled to receive one reimbursement for lenses purchased in a designated two-year contract period.
The current two-year contract period is from July 1, 2007, to June 30, 2009.
Reimbursement
- Obtain an original itemized receipt for the purchase of corrective lenses
- Complete a Vision Care Plan Claim Form
- Mail the entire Vision Care Plan Claim Form and the original itemized receipt to University Human Resources
- Your
reimbursement check will arrive 3-4 weeks from the date Human Resources receives the claim
