Open Enrollment 2011
Overview
Chapter 78, P.L. 2011stipulated that the SHBP form committees to review and offer additional health plans with lower premiums in exchange for higher cost sharing when health services are received. The State Health Benefits Program (SHBP) review committee has finalized the additional health plan options for the 2011 Open Enrollment Period. The committee has selected twelve additional health plan options to offer state employees during the annual Open Enrollment period. As a result, the State Health Benefits Program (SHBP) has designated October 17, 2011 to November 11, 2011 as the annual Open Enrollment period for the State Health Benefits Program and New Jersey Tax$ave program.
During Open Enrollment, you will have the opportunity to assess your benefits needs and revise your coverage. If you do not wish to change your health coverage, no action is required. The Tax$ave plans do not renew automatically, therefore, you must enroll or reenroll during the Open Enrollment period to participate in the Section 125 Flexible Spending Accounts in 2012. All changes made during Open Enrollment will become effective January 1, 2012.
If You Do Not Want to Make Changes to Your Current Health Plan:

To participate in the Tax$ave unreimbursed medical and/or dependent care flexible spending accounts (FSA) program for 2012; you must submit your election directly to FBMC no later than November 11, 2011. You must enroll or re-enroll in the Tax$ave program each year to participate in flexible spending accounts. Eligible employees may enroll online at http://www.myFBMC.com, by phone at 800-865-FBMC (3262), or by faxing a completed FBMC Spending Account Election Form to FBMC at 850-514-5806. Visit the Tax$ave webpage and the FAQs for more information on the program.
Changes to Health Insurance
If you wish to make changes in your health insurance coverage, a State Health Benefit Program Medical application and Dental application must be returned to Benefits Enrollment ASB II Cook Campus, no later than Friday, November 11, 2011.

Health Insurance Premiums
Employee Pre-Tax Medical and Prescription Insurance Premium Estimator
The State Health Benefits Plan (SHBP) requires employees who elect medical and/or prescription coverage to pay the higher of 1.5% of your base contractual salary or a percentage of the medical and/or prescription premium based on your salary and level of coverage.
You may use the Employee Medical and Prescription Insurance Pre-Tax Premium Estimator to estimate the monthly pre-tax premium that you will pay for your medical and/or prescription coverage.
The new insurance premium contribution will become effective on January 1, 2012
