Employment Eligibility Verification Form (I-9)
U.S. Citizenship and Immigration Services released a revised Employment Eligibility Verification Form (I-9) on November 7, 2007. The previous form is no longer acceptable.
Changes to the Form
The updated form should be completed exactly the same way as the previous one was. The only difference is the types of documents that employers may accept in Section 2.

Required Information
All employees must complete Section 1 of the new form within three (3) business days of the date employment begins. Required information: name, address, maiden name, date of birth, social security number, and whether the employee is a U.S. citizen or national, a lawful permanent resident, or an alien authorized to work here until (date).
Common Errors in Form Completion
Employers must sign, date, and certify that they have examined original documents presented by employees. Employers may, but are not required to, photocopy the document(s) presented. These photocopies may only be used for the verification process and must be retained with the Form I-9. Employers must record: 1) document title; 2) issuing authority; 3) document number; 4) expiration date, if any; and 5) employment start date.
Although the Employment Verification Form does not require a copy of an employee's Social Security Card, a copy is required for university payroll purposes. University Human Resources will retain the completed Forms I-9 for three (3) years after the date of hire, or one (1) year after the date the employment ends, whichever is later.
If an employee uses a document that expires for verification, employers must complete Section 3 of a new I-9 form on or before the expiration date.
