Returning Health Insurance Applications

Health insurance enrollment applications must be returned to University Human Resources or to the person in your department that administers HR and payroll matters.

To forward your applications and supporting documentation to UHR:

  • Mail or fax the completed election form to WageWorks (address and fax number on Section 1 of election form).

Premium Option Plan (POP)

  • To waive participation, complete POP Declination Form
  • Return the form to your Human Resources department

Voluntary Savings for Members of ABP

  • Complete Carrier Allocation/Salary Reduction Agreement
  • Obtain unique 403(b) Enrollment Form from state-authorized investment carrier (TIAA participants complete Group Supplemental Retirement Enrollment Form)
  • Return unique 403(b) Enrollment Form directly to carrier
  • Provide a copy of the unique 403(b) Enrollment Form, ABP Salary Reduction Agreement, and ABP Carrier Election and Allocation form to your Human Resources department
